1. Who we are
Trellis Pathway is a physician-led ketamine and Spravato® therapy practice. This policy explains what information we collect from patients, prospective patients, and visitors to this website, how we use it, and the choices you have. Clinical care is provided by our affiliated medical practice, which is the covered entity responsible for your health record under HIPAA.
2. Information we collect
- Information you give us when you complete the Become-a-Patient intake, waitlist signup, provider referral, or contact forms — including name, date of birth, contact details, insurance, presenting concerns, medications, and the clinical history you choose to share.
- Protected health information (PHI) created during evaluation and treatment — diagnoses, vitals, dosing records, peer-monitor observations, clinical notes, and billing records.
- Website usage data — IP address, browser and device information, pages viewed, and referring source, collected through standard server logs and cookies for security and product analytics.
3. HIPAA and your health information
Your treatment record is governed by HIPAA and by the Notice of Privacy Practices you receive at intake. We use PHI to provide care, coordinate with other clinicians involved in your treatment, obtain payment, and run required operations such as quality review, REMS reporting for Spravato®, and audit logging. We do not sell PHI and we do not use PHI for marketing without your written authorization.
4. How we use the information you provide
- To evaluate whether ketamine or Spravato® therapy is appropriate for you.
- To schedule appointments, communicate about your care, and coordinate referrals.
- To verify insurance benefits, obtain prior authorization, and bill for services.
- To meet legal, regulatory, and accreditation requirements, including REMS reporting.
- To secure our systems and prevent fraud or abuse.
5. How we share information
We share information only with: (a) clinicians and staff involved in your care; (b) pharmacies, laboratories, and peer monitors as needed to deliver treatment; (c) your insurer or payer for coverage and billing; (d) technology vendors that host or support our systems under written agreements (including, where applicable, HIPAA Business Associate Agreements); (e) regulators or authorities when required by law; and (f) successors in connection with a sale or reorganization, subject to equivalent protections.
6. Communications and electronic messaging
By submitting a form on this site, you consent to be contacted by phone, email, or text at the contact details you provide, for purposes related to your inquiry and your care. Standard message and data rates may apply. You may opt out of non-clinical communications at any time by replying STOP or contacting us.
7. Cookies and analytics
We use a small number of cookies and similar technologies for session security, form integrity, and aggregate analytics about how this website is used. We do not use third-party advertising cookies to track you across other sites.
8. Data retention
Clinical records are retained for the periods required by state and federal law and by professional standards. Form submissions from prospective patients are retained for a reasonable period to respond to your inquiry and document outreach, then archived or deleted.
9. Your rights
You may request to access, amend, or obtain an accounting of disclosures of your PHI as described in our Notice of Privacy Practices. Depending on where you live, you may also have rights under state privacy laws to access, correct, or delete personal information. To exercise a right, contact us using the details below.
10. Security
We use administrative, physical, and technical safeguards including TLS encryption in transit, encryption at rest, role-based access controls, audit logging, and least-privilege access for staff. No system is perfectly secure; if a breach affecting your information occurs, we will notify you as required by law.
11. Children
This website is not directed to children under 13. We treat adolescent patients only through a formal clinical intake with parent or guardian consent.
12. Changes to this policy
We may update this policy from time to time. The “Last updated” date at the top reflects the most recent revision. Material changes will be highlighted on this page.
