These Terms & Conditions (“Terms”) govern your access to and use of the Trellis Pathway website and the forms and content made available through it (the “Site”). By using the Site you agree to these Terms. If you do not agree, do not use the Site.

The Site is provided for general educational and informational purposes. It is not medical advice and does not create a physician–patient relationship. A physician–patient relationship is established only after a formal intake, in-person evaluation, and acceptance into care by one of our clinicians. Trellis Pathway offers IV ketamine, at-home oral ketamine with peer-monitor support, and Spravato® (esketamine). Spravato® is FDA-approved for treatment-resistant depression and is administered at a REMS-certified site under the federal REMS program. IV and at-home ketamine are administered off-label for mood and pain conditions. All treatment requires a physician evaluation. Results vary and treatment may not be appropriate for every patient.

You must be at least 18 years old to submit a form on the Site on your own behalf. Adolescent care is initiated only with a parent or legal guardian. By submitting any form, you represent that the information you provide is accurate and that you are authorized to share it.

When you submit a Become-a-Patient intake, waitlist signup, provider referral, or contact form, you authorize us to contact you by phone, email, or text at the contact details you provide, for purposes related to your inquiry and your care. Standard message and data rates may apply. You may opt out of non-clinical communications at any time. Submitting a form does not guarantee acceptance into care, an appointment, or any particular treatment.

Pricing displayed on the Site is current as of the date shown and may change. Spravato® is typically billed to insurance and we handle prior authorization where required. IV and at-home ketamine programs are generally cash-pay at flat rates published on the pricing page. Your final cost depends on your benefits, authorizations, and the plan your physician recommends. Deposits and cancellation terms, if any, are disclosed before you book.

You agree not to: (a) submit false, misleading, or another person’s information without authorization; (b) interfere with or disrupt the Site or its security; (c) attempt to access accounts, data, or systems you are not authorized to access; or (d) use the Site to send unlawful, harassing, or harmful content.

Your use of the Site is also governed by our Privacy Policy, which describes how we collect, use, and protect your information, including protected health information under HIPAA.

The Site, including its text, design, graphics, logos, and the “Trellis Pathway” name and marks, is owned by Trellis Pathway or its licensors and is protected by intellectual property laws. You may view and share pages for personal, non-commercial use. You may not copy, modify, distribute, or create derivative works without our written permission.

The Site may link to third-party websites or reference third-party services for your convenience. We do not control and are not responsible for their content, policies, or practices.

The Site is provided on an “as is” and “as available” basis. To the fullest extent permitted by law, Trellis Pathway disclaims all warranties, express or implied, including merchantability, fitness for a particular purpose, and non-infringement. We do not warrant that the Site will be uninterrupted, error-free, or free of harmful components, or that any information on it is complete or current.

To the fullest extent permitted by law, Trellis Pathway and its owners, clinicians, and staff will not be liable for any indirect, incidental, special, consequential, or punitive damages arising out of or related to your use of the Site. Nothing in these Terms limits liability that cannot be limited under applicable law, including liability for clinical care provided under a formal patient relationship, which is governed by the consents you sign at intake and by applicable medical-malpractice and professional-licensing law.

You agree to indemnify and hold harmless Trellis Pathway from any claims, damages, or expenses arising out of your misuse of the Site or violation of these Terms.

These Terms are governed by the laws of the state in which Trellis Pathway is organized, without regard to conflict-of-laws rules. Any dispute relating to the Site will be brought in the state or federal courts located in that state, and you consent to that venue.

We may update these Terms from time to time. The “Last updated” date above reflects the most recent revision. Your continued use of the Site after changes are posted constitutes acceptance of the updated Terms.
